Dear [Mr./Mrs.] [Last Name of Recipient]:
On [date goods were tendered] you made a tender of [type of goods] in performance of our agreement dated [Date of agreement]. On [First Date of Notification] I notified you that the goods did not conform to the agreement as follows: [Reason for nonconformity], but that I would nevertheless accept the goods if you would promptly cure the nonconformity by taking the following actions: [Description of actions].
By letter dated [Date of recipient's letter], you assured me that you would remedy the defect in the very near future and in reliance on that assurance I accepted the goods, notifying you of that fact by letter dated [Date of my first letter].
I have not heard from you since that time. Since more than a reasonable time has already elapsed and you have not cured the nonconformity in any way, I hereby revoke acceptance of the goods pursuant to [Citation of local enactment of UCC $ 2-607.2].
